How do I apply?

Complete and submit the DBE Program Uniform Certification Application (Application), Personal Financial Statement (PFS) and supporting documents to:

All items on the Application or Personal Financial Statement that are not applicable should be indicated as “N/A”.

The information requested on the Application and PFS is necessary to determine whether your firm qualifies as a DBE according to the guidelines established by Title 49, Code of Federal Regulations (CFR), Part 26. These regulations may be viewed at the following web-site: http://osdbu.dot.gov/DBEProgram/index.cfm.

For firms seeking initial certification, upon receipt of the required forms and documents, an on-site visit will be scheduled (a typical on-site visit takes 2 hours). On-site visits do not apply to out-of-state firms.
How Will I Know if I’m certified?

Upon completion of the review, you will receive a letter of certification or a denial letter explaining the reasons for the denial and the appeals procedure.

If you are certified, the certification is valid for at least three years. At the end of your certification period, your firm must apply for re-certification. You will be sent a notice approximately thirty (30) days prior to your certification expiration date.
