• Complete and submit the DBE Program Uniform Certification Application (Application), Personal Financial Statement (PFS) and supporting documents to:

• All items on the Application or Personal Financial Statement that are not applicable should be indicated as “N/A”.

• The information requested on the Application and PFS is necessary to determine whether your firm qualifies as a DBE according to the guidelines established by Title 49, Code of Federal Regulations (CFR), Part 26.  These regulations may be viewed at the following web-site: http://osdbu.dot.gov/DBEProgram/index.cfm.

• For firms seeking initial certification, upon receipt of the required forms and documents, an on-site visit will be scheduled (a typical on-site visit takes 2 hours).  On-site visits do not apply to out-of-state firms.

How Will I Know if I’m certified?

• Upon completion of the review, you will receive a letter of certification or a denial letter explaining the reasons for the denial and the appeals procedure.

• If you are certified, the certification is valid for at least three years.  At the end of your certification period, your firm must apply for re-certification.  You will be sent a notice approximately thirty (30) days prior to your certification expiration date.
