Hawaii District Office – Permit Applications
Required Documentation:
- Completed & signed Permit Application
- Vehicle Registration Copy
- Vehicle Insurance (must meet criteria noted in permit agreement) Copy
- Any additional documentation stated in permit agreement
- Keep a copy of the permit agreement portion

Instructions for Online Applications:
- Read & scroll the entire application before you click Start.
- Fill out the application form.
- Click on the buttons (labeled “Click to attach…”) to attach scanned copies of required documentation.
- Electronically sign document and click on blue “Click to Sign” button.
- Enter your email and click blue “Click to Sign” button.
- Check your email to confirm your email address.
The application and any documents you attach will automatically be sent to our office and you will receive a copy at the email address provided. We will review and process the application and contact you if further documentation is required.
Payment in the form of a check, cashier’s check, or money order made payable to Department of Transportation (DOT) Harbors. Payments can be dropped off at the Hilo Office, mail slot on the front office door, or mailed.
You can also pay by credit card online at https://harbors.ehawaii.gov/wf/index.html. A service fee of 5% plus a payment processing fee (about 3% depending on the type of card used) will be added to each credit card transaction. To avoid over/under payment, do not pay until application is reviewed and fee is confirmed.
