Hawaii District Office – Ground Transportation Permit Applications

Instructions for Online Applications:

First Time Applicants need to call the Hilo Office (808) 933-8850 before applying for screening.

Select the type of Ground Transportation (GT) that pertain to your business below. The same vehicle may not be listed on multiple GT applications. If you don’t know, please call the Hilo Office at (808) 933-8850.

  1. Once you start the application, you’ll need to complete, and submit. THERE IS NO OPTION TO SAVE AND SUBMIT LATER.
  2. Read and scroll the entire application before you Start. IMPORTANT TO READ the NOTES on Page 12 of the application.
  3. One application allows 15 vehicles to be listed on the register. For additional vehicles, you will need to do another application.
  4. Must attach all the required documents to process your application. NO PICTURE COPIES, ONLY PDF FILES DUE TO LIMITED ATTACHMENT SIZE. Attachments must be pdf & orientation must be portrait (not landscape). Do not shrink documents such as insurance (COI) and vehicle registration. One document per page but one pdf file to upload. Any missing documents or the procedures regarding attachments will be denied with an explanation, and you must re-submit the application. Include if there are any scheduled autos with the Certificate of Liability Insurance. Please place the vehicle registrations in the same order as the vehicles are listed on the application register.
  5. To process your application, we must have the completed GT application, required Certificate of Liability Insurance (any scheduled autos if there is one), vehicle registration(s) and payment.
  6. Before using online payment and avoid over/under payment, please verify the amount with the Hilo Office at (808) 933-8850. Online payment is optional, not required.
  7. When the documents are completed, you will receive the eSign notification saying: Completed “Permit”. We will not send another email or call you upon completion. Completed means for you to print all the PDF’s and to pay as stated on the application. Once we receipt the payment and issue the permits, you will be called to pick-up the permits (it won’t be on the same day). Hours for GT Applications is M-F, 8 am to 2 pm except Holidays and changes in schedule.
