All permit applications are to be completed and turned in through one of the

Required Documentation:

1. Permit Application and Agreement
2. Vehicle Registration
3. Driver’s License
4. Vehicle Insurance (must meet criteria noted in permit agreement)
5. Payment in the form of a check, cashier’s check, or money order made
payable to:

Department of Transportation Harbors Division

You will be contacted when your application has been approved and decals will be
mailed to you or scheduled for pick up.
