2. APPLICABILITY:

Applicant to obtain a permit to perform engineering survey, maintenance and inspection work only – a purely service type of activity. No digging or disturbing of the highway will be permitted under this permit.

3. PERMIT REQUIREMENTS:

a. Applicant to procure, at his own expense, and keep in force at all times when work permitted under said permit is being done, a policy or policies of public liability and property damage insurance, naming the State as a co-insured, covering the entire work to be done under said permit and for at least the minimum coverage set forth hereinafter.

Bodily Injury or Death:
$100,000 minimum limit, one person
$300,000 minimum limit, each accident

Property Damage:
$50,000 minimum limit, each accident

Applicant will submit a certificate of insurance at the time of application.

b. Non-refundable $10.00 fee.

c. Two (2) set of plans.

d. A Lane Closure Form will need to be filled out (two weeks in advance of closure) if proposed work involves land or shoulder closures.
