LANE USE / OCCUPANCY PERMIT
1. WHERE TO APPLY:

2. APPLICABILITY:
Applicant needs a special permit to occupy a lane (within State DOT Highways jurisdiction) for construction activity adjacent to or within the DOT Highways right-of-way.
3. PERMIT REQUIREMENTS:
Provide the following information:
a. Applicant’s Name, Organization, Address, and Telephone Number.
b. Location in the highway, from cross road to another.
c. Date and Time of construction.
d. Certificate of insurance. Applicant to procure, at his own expense, and keep in force at all times when work permitted under said permit is being done, a policy or policies of public liability and property damage insurance, naming the State as a co-insured, covering the entire work to be done under said permit and for at least the minimum coverage set forth hereinafter.
Bodily Injury or Death:
$100,000 minimum limit, one person
$300,000 minimum limit, each accident
Property Damage:
$50,000 minimum limit, each accident
Applicant will submit a certificate of insurance at the time of application.
e. Non-refundable $10.00 fee.
f. Two (2) set of plans.
g. A Lane Closure Form will need to be filled out (two weeks in advance of closure) if proposed work involves land or shoulder closures.
4. REVIEW PROCEDURE:
There is a minimum fourteen (14) working days prior to the date of the construction. Apply at the Oahu District Office in person. Review time varies.
