Request A Copy Of Your Hawaii Driver History Record
You may obtain a copy of your Hawaii Driver History Record (DHR) by visiting one of the District Courts or by writing to any of the District Courts listed below. The DHR is intended for operators of commercial vehicles to keep track of and show all of the CDL disqualifying conviction types as specified in FMCSA rules. For non-commercial vehicle operators, the DHR will show the driver’s citation and preliminary conviction and may not always reflect the final court disposition. A traffic abstract or traffic court report must be obtained to reflect any conviction that was subsequently overturned. The fee for a DHR is $9. See the Request for Driver History Record form which include instructions on filling out, printing and submitting the fillable pdf form.
