2. APPLICABILITY:

Applicant required to have a permit to temporarily park on select shoulders or auxiliary lanes of a restricted section of highway under the State DOT’s jurisdiction.

3. PERMIT REQUIREMENTS:

Provide the following information:

a. Applicant’s Name Address and Telephone Number (organization, if any).
b. Location on the highway, from one cross road or address to another.
c. Date and Time of the occasion.
d. No fee required.
